From Queens, NY via I-495 W: Get on I-495 W from Utopia Pkwy and Horace Harding Expy. Continue on I-495 W to Manhattan. Take the exit toward Downtown from I-495 W. Turn right onto E 35th St and turn left at the 2nd cross street onto Lexington Ave. Turn right onto E25th St, and Manhattan Primary Care will be on your right.

From Brooklyn, NY via I-278 E: Get on I-278 E from Pacific St and Bedford Ave. Continue on I-278 E and take I-495 W and Queens Midtown Tunnel to E 35th St in Manhattan. Take the exit toward Downtown from I-495 W. Turn right onto E 35th St and turn left at the 2nd cross street onto Lexington Ave. Turn right onto E 25th St, and Manhattan Primary Care will be on your right.

From the Bronx, NY via FDR Dr: Take Morris Park Ave and Sheridan Blvd to I-278 W. Continue on I-278 W and take FDR Dr to E25th St in Manhattan. Take exit seven from FDR Dr and turn right onto E 25th St. Turn right onto 3rd Ave, turn left at the 2nd cross street onto E 27th St. Turn left onto Lexington Ave and turn right onto E 25th St. Manhattan Primary Care will be on your right.
